MooreHaven was founded in * to provide a day
service for adults with mild to moderate intellectual disabilities.
Service Users attend from within a *-mile
radius of Tipperary Town, in counties Tipperary and Limerick, and have
grown from an initial four in * to about
* today. MooreHaven is an integral part of
Tipperary and employs up to * full- and
part-time skilled service providers. Services provided have grown
significantly over the years and now encompass Day Activities,
Supported Work/Employment, Day Services, Training Programmes, and
HIQA-registered Residential & Respite facilities.

OUR MISSION is to promote and support independent living and community
integration, personal development and work opportunities for people
attending the Service.

WE ARE CURRENTLY RECRUITING FOR THE FOLLOWING POSITION

HUB CO-ORDINATOR 

(O’BRIEN STREET, TIPPERARY) 
FULL TIME PERMANENT CONTRACT. (MINIMUM **APPLY ON THE WEBSITE** HOURS
ON THE FLOOR)

The purpose of this role is to co-ordinate day service hub activities
on a day to day basis and provide direction and support to staff to
improve the everyday lives of service users.

The hubs must operate under the New Direction guidelines which aim to
promote a person centred needs led approach.

The hub co-ordinator will aim to support individuals with complex
needs, promote a healthy independent lifestyle, and lead a team
striving to deliver the best service to the service users.

The Hub Co-ordinator will require leadership skills with the ability
to support both staff and service users. The person will report to the
day service manager with support from other senior managers.

DUTIES OF THESE POSITIONS INCLUDE:

* Lead and ensure each person supported has an up-to-date and
meaningful person-centered plan with clear support needs and goals
identified and regularly reviewed both short and long term.
* Guide, direct and supervise staff in the implementation of goals
identified in each person's plan and in the delivery of support.
* Carry out 1:1 support and supervision meetings with staff to
provide feedback and deal with staff issues. Ensure these issues are
effectively communicated to relevant managers.
* Ensure the hub is adequately staffed on a daily basis and cover is
arranged as required.
* Promote a positive workplace culture of active listening and
engagement with staff.
* Support the hub team in developing and implementing programmes and
timetables in line with services assessed needs and wishes.
* Assist the hub team to develop and support the implementation of
personal care and supports plans and risk assessments in line with the
assessed needs of the service users. Ensure the regular review of
these plans
* Ensure the correct procedures are adhered to relation to the
collection, storage, administration and safekeeping of drugs and that
medical reports and records are kept/completed. Administer medication
as required.
* Develop and maintain effective communication systems both formal
and informal with service users, families, residential services etc.

ESSENTIAL REQUIREMENTS:

* Professional qualification in Social Work/Applied Social Studies
OR other relevant level 7 qualification.
* A qualification in People Management and/or a Leadership
qualification.
* 3 years relevant work experience in the intellectual disability
sector.
* Supervisory or management experience in the area of health and
social care.
